Fixed: duplicate phase showing up after setup

Course correction

Some people finished onboarding and found two entries in their Phase History when they'd only chosen one. That's fixed.

What changed

• Setup now saves your profile and starting phase together in a single step, so your history shows exactly what you chose.
• New setups record cleanly. If you're already set up, you don't need to do anything.

The engineering side

• The onboarding RPC (complete_onboarding) was extended to a 13-argument atomic write. Before, the trigger and the onboarding write could each create a phase row.
